      Stop by the office and give us your email address

2.   An invitation is sent to your email to set up chart access.

      Have your birthdate, your name as it appears in clinic 

      records and.....

​3.   The login and password are set up on this first time in the system 

4.   You can access information in your chart, request an appointment,

      request a medication refill, send a note to your provider or the 

      clinic staff, access your immunization record and more! 

​5.   You can also get access to another person's chart if they invite you.
